Curriculum & Instruction Graduate Forms

The following links provide helpful information for graduate candidates.ÌýFollow the steps outlined below to begin completing the process. Be sure to submit typed documents with the proper number of credit hours, course titles & names, as well as correct catalog year.Ìý

  1. Use the Graduate Degree Plan MASTER template found below.Ìý You will transpose some ofÌý this information into the university template.
  2. Using theÌý new university template , include your name, WIU ID, current mailing address, major and option/emphasis, oldest graduate course semester and year, as well as the catalogÌýyear of your oldest course. For example: If you began in the summer of 2019, you will use the catalogÌýyear of the previous year (2018-2019).
  3. From the Graduate Degree Plan MASTER template, cut and paste your courses into the universityÌýtemplate form. Be sure to include the most current grades and semester you completed the courses. If you have more than 10 courses, you can include them as well.
  4. If you do not see a course you have taken in the template, use theÌý Ìýto locate the correct course listing, title and description. Use the Graduate CatalogÌýfor other course, such as those with the EDS designation.Ìý
  5. Once you have completed your Degree Plan, date it & email it to Dr. Sebastian Szyjka (SP-SZYJKA@wiu.edu). After it is received, it will be sent through Adobe Acrobat Reader for your digital signature, afterwhich it will be forwardedÌýto the graduate school. Do not forget to use the Graduate Degree Plan MASTER template below so you can populate the correct course prefix and descriptions.Ìý
